If enrolling as an Auxiliary Member, you can skip the REQUIRED form fields:
School Name, Street Address & Phone Number,
as this info is not needed from Auxiliary Members.

However, please write in the word “VOID” as these fields ARE required
and therefore the form will fail to submit if left blank.
